# Create Assessment

Creates a new assessment for a client.

## Parameters

  | ---------- | ----------------------------------------------- |
  | Client ID  | The unique identifier of the client.            |
  | Name       | The name of the assessment.                     |
  | Notes      | Additional notes to be added to the assessment. |
  | Start Date | The assessment start date.                      |
